Netcare is embarking on a credentialling process and updating the personal information of all independently contracted healthcare workers (ICHW).
The information that you provide will be used for the purpose of credentialling and to enhance your profile on the Netcare website.

By providing this information you consent to Netcare storing your information in a central database and processing
your information for the purpose of credentialling and for specified business programmes (e.g. CareOn^TM, SmartSearch^TM etc).
All reasonable steps as required in law will be taken to protect the data from unauthorised access, loss or compromise of data.

Please note:

All fields marked with a * are mandatory and are used for the purpose of credentialling. All other fields are not mandatory, however, should you wish to enhance your website profile, please complete the fields marked with (Website). If you do not want this information displayed on the website, do not complete these fields.
Anaesthetists, locums, surgical assistants and ED doctors only need to complete the mandatory fields.
None of the documents you upload will be displayed on the website.

Some helpful tips:

Please check your information before clicking "Submit" at the end of the last page as you will not be able to go back and edit information that is already captured.
You can leave and return to the form as needed. If you would like any page saved, click “Next Page” before you leave the form.
If you change or update any information on a page, please select "overwrite saved data" or "overwrite saved document" in order for the changes to be saved.
It is advised that you have scanned copies of the following documents ready for uploading before you start

Mandatory:
- ID or Passport (Certified PDF)
- Council Registration Document (PDF)
- Proof of Primary Health Sciences Qualification

Optional:
- Website Profile Picture (2MB)
- RWOPS permission letter
- Proof of any Additional Health Sciences Qualifications
- SASA/Netcare/CSP Agreement (signed) (only applicable to anaesthetists)

This form should take no longer than 20-30 minutes to complete.
By submitting this form, you declare that all information provided is true and correct.

Council Registration Number:

(e.g. HPCSA, Pharmacy Council, etc.). Please capture as it appears on your council registration
document including the prefix, without spaces between the letters and numbers.)
Nurses and Social Workers to have no prefix or special characters.
